Surgeon
Perk: Gain sanity and a short speed boost after healing a patient.
If you’re looking for the strongest class overall, Surgeon is the easiest recommendation. The combination of sanity recovery and bonus movement speed helps throughout an entire shift.
The speed boost might not sound impressive on paper, but once the hospital becomes hectic and patients start stacking up, being able to move between rooms faster saves a surprising amount of time. The sanity recovery is also consistently useful since you’ll constantly be healing patients.
Unlike some classes that only help in certain situations, Surgeon provides value every single game.
Secret Agent
Perk: Start with a gun containing 20 uses.
The Secret Agent is one of the strongest premium classes available. Starting with a weapon immediately gives you a solution for dangerous situations and certain hostile encounters.
While the gun isn’t necessary to complete shifts, having access to it from the start removes a lot of pressure during difficult runs.
The only real downside is that it’s locked behind Robux, making it inaccessible for many players.
A Tier Classes
Head Nurse
Perk: +3 Maximum Inventory Capacity.
Inventory space is far more valuable than many players initially realize.
As you progress, you’ll constantly carry medical supplies, tools, coffee, and other useful items. Having three extra inventory slots gives you significantly more flexibility and reduces the need to constantly manage your inventory.
It’s not flashy, but it’s one of the most practical perks in the game.
Security
Perk: Start with a taser with 5 uses.
Security starts every shift with a taser, which can be incredibly useful for handling dangerous situations.
The taser offers valuable utility without requiring additional preparation, making it one of the strongest early-game perks available.
While five uses isn’t a massive amount, it is usually enough to handle several emergencies before needing alternative solutions.
Nurse
Perk: +1 Maximum Inventory Capacity.
Nurse deserves a higher ranking than many players initially give it.
The perk itself seems simple, but the class is extremely affordable and immediately useful. One extra inventory slot doesn’t sound like much, yet it makes carrying supplies noticeably easier during longer shifts.
For beginners, this is probably the best first class purchase in the game.
I actually found myself getting more value from Nurse than some of the more expensive sanity-focused classes simply because inventory space is always relevant.
B Tier Classes
Psychologist
Perk: All sanity effects are doubled, both positive and negative.
Psychologist is easily the most unusual class in Animal Hospital.
Doubling positive sanity gains can be powerful, especially if you manage your sanity efficiently. Unfortunately, the perk also doubles negative sanity effects, making it a risky choice.
In the hands of an experienced player, Psychologist can perform very well. For newer players, however, it often creates as many problems as it solves.
C Tier Classes
Doctor
Perk: Recover 1 sanity when healing patients.
Doctor sounds better than it actually feels during gameplay.
Recovering sanity while healing patients is useful, but the effect is fairly minor. Since sanity can often be restored through other methods, the perk doesn’t have as much impact as inventory or mobility bonuses.
It’s certainly playable, but there are simply stronger options available.
Secretary
Perk: Recover 1 sanity when checking in patients.
Secretary offers almost the same type of benefit as Doctor, except it activates during patient check-ins instead.
While the perk works consistently, sanity management rarely feels difficult enough to justify choosing this class over stronger alternatives.
Once you become familiar with the game systems, you’ll probably stop noticing the perk altogether.
Paramedic
Perk: Start with a Large Speed Cooler with six uses.
The Paramedic begins each run with a useful item, which sounds promising at first.
Unfortunately, the item doesn’t offer enough value compared to classes that provide permanent bonuses. Once the cooler’s charges are gone, you’re essentially playing without a class perk.
It’s not terrible, but it’s hard to recommend over stronger options.
D Tier Classes
Intern
Perk: Start with 10 bonus sanity.
Intern is the default starter class, and unfortunately it feels exactly like one.
The extra sanity is nice during your first few games, but it quickly becomes irrelevant once you learn how sanity management works.
Compared to every other class in the game, Intern simply offers the weakest long-term value.
If cost isn’t a concern, Surgeon is currently the strongest overall class in Animal Hospital thanks to its combination of sanity recovery and movement speed. The perk remains useful from the start of a shift all the way to the end, making it the most reliable option available.
For free-to-play players, Nurse provides the best value and should be your first priority. Its inventory bonus might seem small, but after several hours of gameplay you’ll quickly realize how useful that extra slot really is.
At the end of the day, class perks are helpful rather than game-changing. Skill, efficiency, and knowing how to handle patients matter far more than any class bonus. Still, picking one of the stronger classes can definitely make your life in Animal Hospital a lot easier.
