This guide breaks down the best weapons and skills in Cell Survivor Shoot Defense, based on how they actually perform across vertical stages and round (circular) stages. Some early skills completely outscale late unlocks, and a few “legendary” options are honestly traps.
Cell Survivor Tier List 2026: Best Weapons & Skills
S Tier
These skills carry runs by themselves and work in nearly every stage type.
Cotton Swab (Sterile Swab)
This is the best all-around skill in the game. It stays active forever, does constant damage, and works just as well in vertical stages as round ones. It scales beautifully and never feels wasted.
If you only remember one thing from this guide, it’s this: Cotton Swab is S tier everywhere.
Medical Tower (Laser Tower)
Slightly less raw damage than Spinning Daggers, but much more consistent across different map shapes. The lasers control space, thin waves, and give you breathing room when things get messy.
Borderline S+, but safely S-tier for consistency.
S– Tier
Spinning Daggers
In round stages, this skill is basically cheating. Enemies melt the moment they get close. In vertical stages, it loses some value, but still acts as excellent personal defense.
If your run has a lot of circular maps, this jumps to S+ instantly.
Acupuncture Needles
Extremely underrated. Once upgraded, you’re firing multiple needles that split and hit repeatedly. They’re easy to aim, scale well, and work equally well in both stage types.
One of the most reliable damage dealers in the game.
A Tier
Excellent splash damage, especially in vertical stages where lining enemies up is easy. At low levels the damage feels weak, but it scales very well.
Not mandatory, but absolutely strong if you have access to it.
Disinfectant Bubble
Almost S-tier in round stages. The bouncing damage stacks up fast when enemies surround you. In vertical stages, some bounces get wasted on walls, which holds it back slightly.
Still a very solid pickup.
Special Effects Tablet
Good impact damage plus burn damage over time. Shines more in vertical stages than round ones. Not something you build around often, but it pulls its weight when upgraded.
Tadpoles
Simple, flexible, and effective. They don’t last as long as Snot Dragon, but you can fire many more of them. Scales well and works in all map types.
B Tier
Snot Dragon
Great beginner skill. Auto-targeting, flies around, and helps early runs a lot. Once you unlock stronger options, it slowly falls off.
Still fine if you get it early.
Massage Sticks
Average damage early, but quantity and crit upgrades can make them respectable. Much better in vertical stages than round ones.
Not bad, just overshadowed later.
Hyperostosis (Bone Spikes)
Decent damage over time and good area denial. Starts weak but scales nicely. Doesn’t dominate, but never feels useless.
A-Hair (Whip)
Big splash hits and no aiming required, but no sustained damage. Hits hard, then disappears. Ends up doing less total damage than it looks like it should.
Fine, but replaceable.
C Tier
Large Scalpel
High single-hit damage, but no damage over time and poor scaling. The melee portion helps a bit, but overall it doesn’t justify a slot.
Roulette
One of the most disappointing skills in the game. Long spin time, random results, and zero consistency. Even when it gives you something good, the delay kills its value.
Legendary in color only.
Defibrillator
Random targeting, no splash, no DoT, inconsistent damage. It rarely hits what you want and never enough to matter.
One of the weakest skills available.
